THESE DOCUMENTS MUST BE TURNED IN BEFORE AN ATHLETE CAN TRYOUT or CONDITION FOR A WMS ATHLETIC TEAM.

Please download the packet below or stop by the front office to retrieve a copy.

Forms not COMPLETELY filled out will be returned to the athlete, and he/she will not participate in WMS athletics until corrected.

All forms will be kept by the WMS Athletic Director.
